This Notice of Privacy Practices describes how medical information about you may be used and disclosed, and how you can access this information. This notice is provided in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

Our Commitment to Your Privacy
Hebdon Chiropractic & Wellness is committed to protecting the privacy of your protected health information (PHI). PHI includes information that identifies you and relates to your past, present, or future physical or mental health condition, the healthcare services you receive, or payment for those services.
We are required by law to:
  • Maintain the privacy and security of your PHI
  • Provide you with this Notice explaining our legal duties and privacy practices
  • Follow the terms of the Notice currently in effect
This Notice applies to all records created or maintained by our practice.

How We May Use and Disclose Your PHI
Treatment
We may use and disclose your PHI to provide, coordinate, or manage your healthcare. This includes sharing information with other healthcare providers involved in your care, such as referral providers or specialists.
Payment
We may use and disclose your PHI to bill and collect payment for services you receive. This may include communication with insurance companies or other third parties responsible for payment.
Healthcare Operations
We may use and disclose your PHI to support the business operations of our practice, including quality assessment, training, compliance, and administrative activities.
Appointment Reminders and Office Communications
We may call your name in the waiting area, contact you to remind you of appointments, or communicate with you regarding your care by phone, voicemail, text message, email, or mail.
Electronic communications may not be fully secure. You may request alternative methods of communication at any time.
 
Uses and Disclosures Required by Law
We may disclose your PHI as required or permitted by law, including for public health reporting, abuse or neglect reporting, legal proceedings, workers’ compensation claims, and to prevent a serious threat to health or safety.

Special Confidentiality Protections (42 CFR Part 2)
In limited circumstances, our practice may receive records related to substance use disorder treatment that are protected under federal law (42 CFR Part 2). When applicable, such information is handled in accordance with federal confidentiality requirements.

Your Rights
You have the right to:
  • Inspect and obtain a copy of your PHI, including electronic records, generally within 30 days of your request
  • Request corrections to your PHI if you believe it is incorrect or incomplete
  • Request restrictions on certain uses or disclosures (we are not required to agree to all requests)
  • Request confidential communications by alternative means or locations
  • Receive an accounting of certain disclosures of your PHI
  • Receive a paper copy of this Notice at any time
  • File a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services without fear of retaliation
